Its about someone who is mentally ill in a hospital like the whole "tell me that you're alright,

is one of the nurses coming in to his room and checking on him.

And "give me a reason to end this discussion,
to break with tradition.
To fold and divide." he obviously has ocd and multiple personalities. He doesn't want to talk about it anymore but he also wants to stop feeling the way he is

talking with strangers, waiting in line..
I'm through with these pills that make me sit still."
he hates doing things that normal people do because he feels so out of place.. And the dr's give him pills to help him but he's sick of taking them.

"i'm sick of the things I do when I'm nervous
like cleaning the oven or checking my tires
or counting the number of tiles in the ceiling..
Head for the hills, the kitchen's on fire!" he has nervous habits and he hates them but he can't stop.

"I used to rely on self-medication,
i guess I still do that from time to time.
But I'm getting better at fighting the future,

yes, I'll be just fine."
he used to be a lot worse, and the dr's are reasurring him that he's getting better.. And so he tells himself yes I'll be just fine.

his multiple personalities are saying two different things. The one that you hear more clearly in the song...(the one not in parentheses) is the stronger one and the one he is saying out loud. The other voice you hear in the backround.. Is the one in his head and it's thinking about suicide..(the long goodbye) because its just tired of living like this.
