In essence, the song is about a one-sided love affair between GaGa and a best friend of hers. The problem is she wants all of him- the good, the bad, the pleasant, and the nasty, and all the time. He wants some of her, sometimes.

It sort of takes away the romanticism of the "best friends" label. She wants the relationship to propel to new heights but her emotions are bound because she's "just a friend." She parallels the mental torture of her sealed love with physical torture in her video- she's a sex slave. She offers herself to the mobboss's whim just as she offers her emotional self to her best friend. The issue in GaGa's life, however, was that she was not having these feelings reciprocated as referenced in her lyrics:

I want your ugly, I want your diasease. I want your love, I don't wanna be friends- I want your everything... etc.

In the video, the issue was that she been stripped of her sense of autonomy. By the end, however, she kills the mobboss (ignites him with her sex) and becomes headbitch mobqueen.
