He is describing some sort of depression, and people who are close to him saying it's nothing, but he is saying - this is serious, this is affecting my life. And while he is famous, he is saying that inside he is actually a monster, he is messed up. And he desperately wants to get out of this situation, to feel good, but he feels like he can't get away.
